Contract Covers	This contract is intended to cover the state’s normal requirements for drug testing products and services through the Minnesota Multi-State Contracting Alliance for Pharmacy (MMCAP) Infuse cooperative purchasing agreement, including the following goods and services: Breath testing devices Cards and cassettes Dipsticks Combination collection and test cups Additional related products Laboratory Collection
Contract Type	This is a convenience contract for state agencies, departments, institutions, universities, community colleges, and non-mandatory entities including public school systems and local governments.
Special Note	Agencies must be registered as an MMCAP Infuse Participating Member Facility in order to use this contract. If your agency is not currently registered, please complete an MMCAP Infuse Member Application and email it to the Contract Manager. If you have questions or are unsure about your agency’s membership status, please contact the Contract Manager.

Transportation Charges	Vendors shall complete delivery to the buying entity after receipt of purchase order within the following timeframes: Redwood Toxicology Laboratory Inc. Products: FOB destination within 5 to 7 business days Testing Services: Next day air service of inbound specimens sent to vendors, or its affiliates, for laboratory testing is provided at no charge when five (5) or more urine and/or oral fluids specimens are sent in each FedEx overnight shipment. Any combination of urine and/or oral fluids devices may be shipped together via FedEx overnight service. Fewer than five (5) specimens sent to the lab by next day air service will be assessed as a seven-dollar ($7.00) charge per shipment. Premier Biotech Inc. Products: FOB destination within 5 business days Testing Services: Vendors will utilize its own specimen collection facilities when applicable and utilize network contracted sites when necessary to provide collection services for MMCAP Infuse members. The vendor’s collection network includes over 9,000 collection sites that provide an array of specimen collection services and meet the highest standards in collections, including DOT collections. Should an instance arise where the Premier Biotech network does not service an MMCAP Infuse member’s area, the vendor will subcontract with an appropriate agency able to service their location. Vendors will work with other nationally recognized firms to provide the most extensive collections network available. Vendors will coordinate collections directly for MMCAP Infuse members utilizing its own collection sites or those provided by ARCpoint Occupational Solutions (AOS). AOS will provide organizational structure information about these collection agencies once the specific geographical areas and MMCAP Infuse member details have been secured. In the normal course of business, vendors will coordinate with AOS to determine appropriate site matches.
